We are seeking a highly motivated and entrepreneurial Palliative Care Physician to launch and lead a home-based palliative care practice in Georgia. The ideal candidate will have a strong background in palliative care, a passion for improving the quality of life for patients with serious illness, and the business acumen to start and grow a practice that delivers compassionate, patient-centered care in the comfort of patients' homes. This is an exciting opportunity to build  a healthcare model in a new market, shape patient care, and make a lasting impact in the community.

As a Palliative Care Lead Physician (MD), you will:

  1. Clinical Care:
    • Lead and provide comprehensive palliative care to patients in their homes, focusing on pain and symptom management, improving quality of life, and offering emotional and spiritual support.
    • Perform in-depth assessments of patients with serious or life-limiting conditions, develop personalized care plans, and manage their medical needs.
    • Work closely with patients and their families to establish goals of care and ensure treatment aligns with their wishes, emphasizing patient autonomy and dignity.
    • Collaborate with, and clinically support, a team of nurse practitioners as the practice grows.
  2. Entrepreneurial Practice Development:
    • Establish, grow, and manage a successful home-based palliative care practice.
    • Develop partnerships with hospitals, nursing homes, home health agencies, and other healthcare providers to generate referrals and expand service reach.
    • Create and refine business strategies, ensuring a sustainable practice while maintaining high-quality patient care.
    • Hire, train, and manage additional healthcare staff (i.e. nurse practitioners) as the practice grows.
  3. Interdisciplinary Collaboration:
    • Work in collaboration with a multidisciplinary team (nurses, social workers and other healthcare professionals) to ensure a holistic approach to patient care.
    • Provide leadership and mentorship to support the team in delivering compassionate and effective care.
  4. Quality Improvement
    • Stay up-to-date with palliative care guidelines, quality measures, and industry best practices.
    • Implement and monitor processes for quality assurance and continuous improvement in patient care and business operations.
    • Collaborate with the practice Medical Director and Practice Manager in efforts of:
      1. Continuous quality and process improvement, including those related to the EHR and Clover Assistant.
      2. Ongoing evolution of the care delivery and operational models.

Success in this role looks like:

In the first 90 days, you will understand our systems and processes and begin to build rapport with patients, families and the team. Success includes:

  • Learning our technology and platforms to effectively and efficiently deliver care for your panel of patients.
  • Establishing relationships with the interdisciplinary team and understanding the role of each team member.
  • Demonstrating effective communication and collaboration skills with patients, families, and the care team, especially in care planning and patient discussions.

In the first 6 months, you should be fully integrated into the clinical environment and have developed a deeper understanding of your patient population. Success looks like:

  • Actively pursuing patients appropriate for palliative care services into the practice.
  • Building strong relationships within the community to identify collaborative opportunities.
  • Demonstrating the ability to effectively identify, enroll and transition eligible patients to hospice. 
  • Contributing valuable insight during IDT rounds, particularly around symptom management, prognostication, communication skills, and complex end of life decisions. 

In the future, you will be delivering consistent, high-quality care, and contribute to the growth of the palliative care program.  Success looks like:

  • Being recognized as a leader and an expert in palliative care, being trusted by colleagues and patients for clinical decision making and symptom management.
  • Strong, consistent clinical outcomes, especially on timely transitions to hospice.

You should get in touch if:

  • You are based in the state of Georgia (required).
  • You have a MD or DO degree and are Board Certified or Board eligible in Palliative Care, Hospice & Palliative Medicine, or a related field (required).
  • You have an active, unrestricted medical license to practice in the state of Georgia (required).
  • You have an active DEA license (required). 
  • You have a minimum of 3-5 years of clinical experience in palliative care, hospice, or a related field. Experience in a home-based care setting and/or value based care is preferred.
  • You have previous experience in starting, managing, or growing a healthcare practice, business, or department is highly desirable. Ability to navigate the challenges of building a new business and adapting to a dynamic healthcare environment.
